Clinical Presentation and Management of Sweet Syndrome: A Case Report

A patient presented with a sudden eruption of painful, red, inflamed lesions across the face and neck—an alarming development that prompted urgent referral to dermatology. The lesions were sharply demarcated, tender to touch, and had appeared rapidly, raising immediate concern for an acute inflammatory or immune-mediated process. Given the patient’s recent medication history, clinicians initially suspected a possible drug-related reaction, and the suspected agent was promptly discontinued as a precaution.

The initial clinical evaluation triggered a comprehensive diagnostic workup. This included a skin biopsy to assess histopathological features, alongside laboratory investigations such as a complete blood count, inflammatory markers, and an extensive immunological panel. Testing for autoantibodies, lupus anticoagulant, and broader serological markers was also performed to rule out systemic autoimmune or connective tissue disorders.

In the meantime, treatment was initiated empirically with oral corticosteroids to control inflammation and relieve symptoms. The response was both rapid and striking. Within 48 hours, the patient experienced significant improvement—pain subsided, erythema diminished, and the lesions began to resolve. This quick therapeutic response provided an early clinical clue pointing toward a steroid-responsive inflammatory dermatosis.

Laboratory results revealed leukocytosis with marked neutrophilia, supporting the presence of an acute inflammatory response. Immunological testing showed positive antibodies and the presence of lupus anticoagulant, though routine serological markers for systemic autoimmune disease remained negative. These findings, while notable, were not sufficient on their own to establish a definitive diagnosis.

The turning point came with the histopathological analysis of the skin biopsy. It demonstrated dense infiltration of neutrophils within the dermis, without evidence of vasculitis—findings characteristic of Sweet syndrome.

Sweet syndrome, also known as acute febrile neutrophilic dermatosis, is a rare but well-recognized inflammatory condition. It is defined by the abrupt onset of painful erythematous papules, plaques, or nodules, often accompanied by fever and systemic symptoms. Histologically, it is marked by a dense neutrophilic infiltrate in the upper dermis.

The exact pathogenesis of Sweet syndrome remains incompletely understood, but it is widely believed to involve dysregulated immune activity, particularly cytokine-driven activation of neutrophils. It is considered a hypersensitivity reaction and has been associated with a variety of underlying triggers.

These include infections, malignancies—especially hematologic cancers—and medication exposure. Drug-induced Sweet syndrome is a significant subset and has been linked to agents such as antibiotics, antiepileptic drugs, oral contraceptives, antihypertensives, vaccines, and granulocyte colony-stimulating factors. In such cases, identifying and discontinuing the offending drug is a critical step in management.

First-line treatment typically involves systemic corticosteroids, which often lead to rapid and dramatic clinical improvement, as seen in this case. Alternative therapies may be considered in patients who cannot tolerate steroids or who experience recurrent disease.

The differential diagnosis in cases like this is broad and includes conditions such as urticaria, allergic or irritant contact dermatitis, drug eruptions (toxidermia), and cutaneous manifestations of autoimmune diseases like lupus. Because of this overlap, accurate diagnosis depends on correlating clinical presentation with laboratory findings and, crucially, histopathological confirmation.

This case highlights the importance of early recognition and prompt treatment of Sweet syndrome, as well as the need for thorough evaluation to identify potential underlying causes. While the condition itself is often highly responsive to therapy, it can sometimes serve as a marker of a deeper systemic issue, making careful follow-up essential.

Ultimately, it underscores a key principle in medicine: when the skin speaks suddenly and loudly, it often reflects processes that go far beyond the surface.
